Signature provides proactive oversight, expert guidance and accountability while you or your family remain primarily responsible for carrying out recommendations. Premier provides active independence management, with Crestwell taking a more hands-on role in coordinating resources, managing agreed next steps and following priorities through to completion.

Your personalized mobility and wellness program evolves with you. During in-home sessions, we assess progress, update your program and provide hands-on instruction to help you perform it safely and confidently. As needs change, we recommend practical adjustments to your routines, environment or outside resources.
LIVE WELL
Stay connected to the people, activities and purpose that make life meaningful.
We monitor social connection, routines, interests and meaningful engagement as part of your overall Independence Plan.
When new opportunities would be beneficial, Crestwell researches local programs and activities based on your interests, contacts organizations to determine fit and availability, and provides your top options so you can choose and arrange what works best for you.
NAVIGATE WELL
Stay prepared, organized and connected as healthcare needs evolve.
Crestwell helps you prepare for physician visits with personalized questions and provides your latest Independence Dashboard so your physician can see how you are progressing outside the medical office.
When helpful, Crestwell can participate virtually in up to one physician appointment per month. Relevant recommendations and updates are incorporated into your Independence Plan and shared with authorized family members to help keep everyone informed.
STAY AHEAD

Weekly check-ins and ongoing professional oversight help us recognize changes, monitor priorities and keep your Longevity & Independence Plan current.
Your progress is formally reviewed through a quarterly Independence Review & Dashboard, with monthly updates provided to authorized family members.

When concerns or new needs emerge, Crestwell helps move agreed solutions forward by coordinating appropriate resources, equipment, home modifications or outside providers and following progress.

Crestwell researches and vets activities, programs and community opportunities based on your interests, goals and preferences.
We present your best options and, once you choose, help make participation happen. Crestwell can manage agreed booking and logistics, coordinate trusted third-party transportation when needed and follow up afterward to ensure the plan translated into meaningful participation.

Help manage the moving pieces of healthcare.
With your authorization, Crestwell can communicate directly with physician offices and providers, obtain and organize relevant information, track recommendations and referrals, assist with scheduling and follow through on agreed next steps.
Crestwell can accompany you to up to two physician appointments per month, helping communicate relevant Independence Dashboard findings, capture recommendations and close the information loop afterward with you and authorized family members.
